This update is about one line in your log book: the concluding diagnosis. It’s the line preceptors read most carefully, and the one earlier versions of ClinicalRecap got closest to right without quite nailing.

What changed

The auto-fill model now reads your program’s scope-of-practice language before it drafts a concluding diagnosis, instead of applying one generic phrasing across every account. An LPN log book and an MSN log book describing the same encounter will now come back with language appropriate to each level, pulled from what you actually wrote in your notes.

We also tightened how the model handles ambiguous cases. Where earlier versions would sometimes commit to a diagnosis your notes didn’t fully support, the current version flags the line for you to complete yourself rather than guessing.

Why it matters for your log book

A concluding diagnosis in the wrong voice is one of the fastest ways to get a note sent back. This update exists to cut that specific kind of rewrite, not to change how much of the drafting you’re responsible for reviewing before you sign.
